TidalHealth is committed to meeting its mission “to improve the health of the communities we serve” by connecting our community to the resources and support they need through our Community Wellness team.
Our team of nurses, social workers and community health workers provide medical and non-medical support to make sure you are getting the care and support you need to live an active, healthy life. Our community health workers help people with chronic health problems connect to community resources.

Community Health Workers
Community health workers can advocate for you and help connect you to services such as:
- Health insurance
- Primary care (a family doctor)
- Preventive care (mammogram, flu shots, colonoscopy, screening tests)
- Food
- Safe housing and home repairs to make your house safer
- Home medical equipment
- Transportation
- Medication
- Health education about chronic diseases
Community Wellness Van
TidalHealth's Community Wellness Van is a mobile medical unit that provides community-based health screenings. Services may include blood pressure, diabetes assessment, referrals and links to primary care, cancer screenings, seasonal flu shots and more. The van visits underserved communities throughout the Lower Eastern Shore and Sussex County, Del., and community events.

SWIFT Program
TidalHealth and the Salisbury Fire Department work together through the Salisbury-Wicomico Integrated FirstCare Team (SWIFT) program to give you support and tools to manage your health.
The team works with you in your home to determine your healthcare needs and connect you with the right services. Participation in this program is voluntary and offered only to Wicomico County residents. The home visits are free to participants.
SWIFT includes a nurse practitioner, community paramedic, registered nurse, social worker, community health worker and other health professionals working together to connect people to the appropriate services.
You may be referred to the SWIFT by the 911 call center, the emergency department or a care provider. A member of our team will contact you to schedule a home visit.
The visit may include:
- Physical exam (checkup)
- Medication review
- Home safety check
- Social needs survey
- Health education
- Discussing goals and ways you would like to improve your health

REACH Program
The REACH (Rural Equity and Access to Community Health) Project is here to help our friends and neighbors prevent and control diabetes and high blood pressure. Services also address social factors that impact health and wellbeing.
Free services include:
- Health programs and presentations for your community
- Classes near you to learn how to live a healthy lifestyle
- Help connecting to community services and resources like food, housing, and transportation
- Help finding health care providers
The REACH Project is made possible through a coalition of partners from:

- Haitian Development Center
- Rebirth, Inc., serving Salisbury
- TidalHealth Community Wellness Department
- Wicomico County Health Department
- Worcester County Health Department
- YMCA of the Chesapeake serving Wicomico County

About the Maryland Community Health Resources Commission
The Maryland Community Health Resources Commission (CHRC) was created by the Maryland General Assembly in 2005 to expand access to health care services in underserved communities, support programs that serve low-income and vulnerable populations, and strengthen the capacity of Maryland’s network of safety net providers across the state through its grant making authority. Since its inception, the CHRC has awarded 866 grants, totaling $291.7 million, supporting programs in every jurisdiction of the State. These programs have collectively served more than 620,000 Marylanders and the grants awarded by the CHRC have enabled grantees to leverage $44.7 million in additional federal, and private/non-profit resources.
