Jennifer Price and Diane Hyde serve as TidalHealth Practitioner Growth Coordinators. They provide administrative and operational support related to the referral process and help ensure referrals are routed appropriately so patients experience continuity of care.

How the referral process works
Referring a patient to TidalHealth is designed to be straightforward, efficient and supportive from start to finish. Our goal is to make it easy for your office to connect patients with the right specialist or service while helping ensure timely scheduling, clear communication and a smooth care experience. From the moment a referral is received, our teams work to coordinate next steps, support the patient through access to care and keep the referring practitioner informed along the way.
Step 1: Submit the referral
Send the referral, supporting documentation, and any relevant clinical information to the appropriate office or service line. If you need help identifying a specialist, please reach out to our practitioner growth coordinators.
Step 2: Our team reviews the information
We review the referral to help ensure patients are directed to the right provider, specialty or level of care
Step 3: We contact the patient
Our team works to schedule the appointment and guide the patient through the next steps.
Step 4: Care is coordinated
Patients receive care from our specialists and support teams with a focus on quality, access and continuity.
Step 5: We communicate back to you
We work to share consult notes, recommendations and follow-up information so you can remain an active part of your patients care.

What if I need to share clinical details?
Please do not send PHI by email or through the form. We will coordinate a secure method if documentation is needed.
