Celebrating Community Health Improvement Week
The American Hospital Association (AHA) Center for Health Innovation and its Community Health Improvement (ACHI) team observe Community Health Improvement (CHI) Week each year to honor those working in partnership to improve the health and well-being of individuals and communities and advance health equity. This year, CHI Week will be observed June 8 to 12.
TidalHealth is committed to its mission “to improve the health of the communities we serve” through a broad range of community health, population health, care coordination, prevention, and outreach initiatives designed to improve access to care, support chronic disease management, address barriers to health, and strengthen overall community well-being.
This work extends far beyond traditional healthcare settings. Across TidalHealth, multidisciplinary teams collaborate with local health departments, schools, community organizations, faith-based partners, EMS, and other regional stakeholders to implement community health improvement strategies, address social determinants of health, and connect individuals and families to the services and support they need.
Improving community health requires collaboration across the healthcare continuum. At TidalHealth, teams in Community Wellness, Care Coordination, Transitions, Care Management, Diabetes Education, the School-Based Wellness Centers, and Behavioral Health work together to improve access to care, strengthen care coordination, address barriers to health, and support healthier communities throughout the region.
