What can I expect at my first visit?
At your first visit, you will meet with one or more of our specialists. We will review your medical information and exams, then recommend any follow-up tests if needed. Our cancer nurse navigator and coordinator are there to answer questions, help with scheduling, and support both you and your family throughout your care.

What should I bring to my first appointment?

  • Photo identification (e.g., driver’s license or state ID)
  • Current insurance card(s)
  • A list of your current physicians, including contact details
  • A detailed list of your current medications (why you take them, the dose amount, and how often you take them)
  • Your pharmacy’s contact information
  • A list of your allergies
  • Medical power of attorney and living will (if available)
  • Any additional information requested by your care team

You may also be sent a packet with forms to fill out before your visit. Please bring these completed forms and any other requested items with you to your appointment.

How does TidalHealth ensure my care is personal?
