Home Healthcare

Home healthcare offers skilled nursing, therapy and personal care services to help patients recover and stay independent in the comfort of their own homes. Caregivers and families receive guidance and resources every step of the way.

At TidalHealth, our home healthcare services are designed to support patients and families in the comfort and familiarity of home. We understand how important it is for you or your loved one to maintain independence while still receiving the necessary care. Our compassionate team works closely with patients and caregivers, providing guidance, encouragement, and practical help with daily needs and treatments.

What We Offer

Our home healthcare services cover a broad range of needs, including:

  • Skilled nursing, where nurses teach and assist you or your caregiver with important care tasks
  • Physical therapy to help improve strength, mobility, and endurance
  • Occupational therapy to support day-to-day activities and increase independence
  • Speech therapy to assist with communication and swallowing needs
  • Medical social work, providing support during the transition from hospital to home and helping families access community resources
  • Programs focused on chronic disease management, wound care, and fall prevention
  • Certified nursing assistants and home health aides to help with personal care as part of your care plan

Personalized Care Plans

Every person’s needs are a little different. Our team works together with you, your family, and your doctor to create a plan of care tailored to your situation. This plan outlines what care and support you need and helps everyone stay on the same page.

Support for Caregivers

We believe in caring for the whole family, not just the patient. Our team offers support, training, and reliable information to help caregivers feel confident and prepared in their role. We are here as a resource, answering questions and offering guidance along the way.

With TidalHealth’s home healthcare services, you can rest easier knowing you have kind, attentive professionals by your side every step of the way.

Who Qualifies for Home Healthcare?

To be eligible for Medicare home healthcare coverage, you must meet these conditions:

  • Your doctor must determine that you need medical care at home and prepare a plan for your care.
  • The care you need should include intermittent, not full-time, skilled nursing care, physical therapy, or speech-language pathology services.
  • You must be considered homebound, meaning you have difficulty leaving home without help and only leave home occasionally or for short periods, often for medical care.
  • The home health agency providing care must be Medicare-approved.

What Services Are Covered Under Medicare for Home Healthcare?

  • Intermittent or part-time skilled nursing care provided by a licensed nurse
  • Home health aide services for help with personal care, such as bathing or dressing
  • Physical therapy, occupational therapy, and speech-language pathology as medically necessary and reasonable
  • Medical social services to provide counseling and connect you with community resources
  • Certain medical supplies like wound dressings
  • Medical equipment, such as a wheelchair or walker (Medicare pays 80 percent of the approved amount)

What Is Not Covered by Medicare for Home Healthcare?

  • 24-hour care at home
  • Prescription drugs you administer yourself
  • Homemaker services like shopping, cleaning, and laundry
  • Personal care, like bathing or dressing, when that is the only type of care you need unless you also need skilled care

What Rights Do I Have as a Home Healthcare Patient?

  • You can choose your home healthcare provider.
  • You will have a single point of contact at TidalHealth to help coordinate your care.
  • Your home healthcare team will work closely with your doctor.
  • Your privacy, dignity, and home will always be respected.

What Is the Difference Between Medicare and Medicare Advantage Plans?

Medicare and Medicare Advantage plans have different benefits and limitations. There is no one-size-fits-all approach, so it is important to review local plan options and discuss with TidalHealth or visit the Medicare website for more details.
