Step 2: Request a Preceptor
We receive many requests from students seeking clinical rotations with TidalHealth Peninsula Regional, TidalHealth Nanticoke and TidalHealth Medical Partners. To better serve you, please complete the Preceptor Request form and return to [email protected] Based on availability, we will advise you if we were successful in matching you up with a preceptor.
Step 3: Submit Agreement and Documentation
Once a preceptor has been established, please print and complete the Preceptorship Agreement to include your signature, a university representative signature and the preceptor signature; if the provider is an Advanced Practice Provider, i.e. Physician Assistant, Nurse Practitioner, Certified Nurse Anesthetist or Certified Nurse-Midwife, ask your preceptor to also obtain their supervising physician signature. Please submit the completed agreement along with all required documentation to Jill North, Medical Education Coordinator at [email protected]. Once all paperwork is received and approved, the Medical Education Coordinator will contact you and issue clearance to begin your clinical rotation. We will make arrangements to obtain a photo ID Badge required to be worn during your clinical rotation.
Step 4: Surgical Orientation (if applicable)
Preceptorships with a surgeon require the completion of an Operating Room (OR) Orientation. Please contact Susan Lynch at [email protected] to schedule. Depending on availability, the orientation may be scheduled for your first scheduled day when planned in advance. Following successful completion of the OR Orientation, Susan will sign the designated area on the Preceptorship Agreement.
Required Documentation for Current TidalHealth Team Members
TidalHealth team members must provide the following documentation to participate in apPreceptorship:
- Two-page TidalHealth Preceptorship Agreement signed by the student, a school representative and the preceptor. If the preceptor is an Advanced Practice Provider (i.e. Physician Assistant, Nurse Practitioner, Nurse Anesthetist or Nurse Midwife), please request your preceptor to also have their supervising physician sign the agreement.
- COVID-19 vaccine
- Copy of student ID
- Copy of TidalHealth name badge
- Copy of student’s nursing license (or nursing license number and state) when completing a clinical rotation related to an advanced nursing program.
- Surgical Preceptorships: documentation of completion of TidalHealth Surgical Services Orientation is required if precepting with a physician with surgical privileges. Contact Susan Lynch ([email protected]) to set up an orientation class. Students may start their surgical rotations on time, however, will not be permitted in the Surgical Services area (OR) until the orientation is complete. It can be scheduled on a day during the student’s first week so they do not have to make a separate trip.
- Complete the highlighted areas of the Information Technology Acceptable Use Policy Acknowledgement and Consent Form along with the checked areas of the Request for Information Services System Sign-On Form (only when the preceptor is requesting IT access.) DO NOT return these forms to the IT Dept. Please include these forms in your submission of documents to the Medical Education Coordinator a minimum of two weeks prior to the rotation start date to allow ample time to create your student account. Please note, you will not be permitted to use your employee log-in while on your preceptorship.
PLEASE NOTE: All students are required to provide their own PPE consisting of Ear Loop Face Masks, N95 masks and face shield. TidalHealth does not allow cloth face coverings.
